COPD

There are thought to be approximately 3 million people in the UK living with COPD, but only around 1.2 million have been diagnosed. 

COPD Admission and Discharge Care Bundles were developed with the aim of improving care and reducing readmission rates for patients with acute exacerbations of COPD, these are available for download below. The Care Bundles are available for use by all clinical teams across the UK. The bundle elements should not be changed but the format of all of the documents can be amended for local use.
